If you have an issue with something internally every company has a way to bring to light things anonymously. As well the govt has ways to discuss things and protect you from the company. Event Description

The user facility observed a false high platelet count (plt) on a burn patient in the emergency room. A complete blood count (cbc) and nucleated red blood cells (nrbc) were analyzed on an xe-5000 automated hematology analyzer at 13:11 in the automated mode. The sample was judged "negative" with no flags. Work area manager (wam) middleware held the report as the plts results were a critical high value. The results were released to the emergency room with a value of plt=770. The operator of the device was unaware at the time that the pt was being treated as a burn pt. The user indicated that at an unspecified time, a phase microscopy performed with plt=105. The original sample was repeated, using two additional channels for a cbc+ differential (diff) +nrbc+reticulocyte at 15:24 with a reported plt=751. The sample results were identified as "positive" with morphology and count errors. A second blood sample was drawn in the intensive care burn unit. The sample was analyzed at 17:10 for cbc and diff +nrbc in the automated mode-plt=611, "positive" with a count error. The pt was removed from life support at an unspecified time and expired.

The customer reported erroneously low analyte results, for one patient, involving the coulter lh 500 hematology analyzer. The erroneous results were released out of the laboratory and questioned by the physician. On (b)(6) 2012, the patient was admitted to hospice bed based on the results. The patient sample was reanalyzed in the manual mode, and the results were acceptable. The customer indicated the laboratory technician released the results in spite the instrument gave partial aspiration each time the sample was analyzed. The customer stated the laboratory technician interpreted the results to be acceptable. A recollection sample was obtained from the patient the following day and partial aspiration was also obtained. The customer sent the patient sample to the main laboratory for further analysis. The main laboratory obtained results that correlated with the clinic's original instrument. This patient is currently doing well and remains in hospice care due to other reasons unrelated to the reported issue. Beckman coulter field service engineer (fse) was dispatched to evaluate the instrument.

Event Description
A customer contacted beckman coulter inc. , (bec) in regards to an erroneously low platelet (plt) result generated by the unicel dxh 800 coulter cellular analysis system, without instrument generated flags, for a single patient specimen. Based on available information, the patient sample was tested for plt earlier on the same day, on a different instrument in the customer lab and a similar low plt result was obtained. The results were reported out of the laboratory. A decision was made to transfuse the patient based on these results which were not confirmed by manual counts. Multiple attempts, for additional information, were made by bec concerning the current condition of the patient; however, this information has not been obtained. The results obtained on (b)(6) 2012 were believed to be erroneous after the customer obtained erroneously low plt results for this patient when the patient returned to the hospital on (b)(6) 2012 after transfusion. This report is covering the event occurred on (b)(6) 2012, instrument s/n (b)(4) mdr #: 1061932-2012-02724 is being submitted for event occurred on (b)(6) 2012, instrument s/n (b)(4). Mdr #: 1061932-2012-02723 covers the event occurred on (b)(6) 2012 at this customer site.

1) what results are we talking about,
2) are they critical or just low,
3) does the rest of the CBC also show low or is it specific for one parameter,
4) normally a hub is used as part of the transfusion criteria and you do not check that parameter manually

Why were the results not checked?. Maybe your laboratory needs to rethink some of its policies. All machines make mistakes....when we use 2 sd as a guidline for qc we are looking at 95.5% accuracy not 100%. A good laboratory and a good Medical Technologist will detect these errors and have policies in place to prevent/detect them. Its not the machine.... Its the man/women
